Employee Health Requirements

  • As a condition of employment, ALL employee candidates are REQUIRED to have a health review and urine drug screen.
  • Job offers may not be extended or may be withdrawn if potential employees do not show up for their appointments.
  • Bring your driver’s license or some type of photo ID for your screening.

Immunization Requirements for Employees

COVID-19

New hires will be asked to provide immunization records of the COVID-19 vaccine. New hires who do not have vaccination documentation will be provided with vaccine education and required to sign an attestation and declination.

Vaccine is optional; new hires are required to provide either vaccination documentation or attestation and declination. 

Influenza

Mandatory ANNUALLY

Meningococcal

One dose for those who are routinely exposed to isolates of N. meningitides (microbiology, pathology, autopsy). Booster vaccine every five years.

Optional

Measles (Rubeola), Mumps, Rubella

Two documented doses of MMR vaccine (no post titer required) or documented immune titer.

Mandatory

Tetanus/Diphtheria/ Pertussis (Tdap)

One adult dose for all staff.

Mandatory

Tetanus/Diphtheria (Td)

One dose recommended every ten years after Tdap and after high-risk situations (bites, needle stick).

Optional

Varicella (chickenpox)

Two documented doses of varicella vaccine (no post titer required) or documented immune titer.

Tuberculosis (TB) Screening

  • All new candidates must have a Quanitferon Gold Lab Assay and TB history review upon hire with Cone Health. Employees with positive results must complete appropriate follow up to rule out active TB including chest x-ray. Health Department referrals will be made in the county of residence for staff with positive Quantiferon Gold.

Urine Drug Screen Instructions

  • Drink plenty of water a half hour before your appointment.
  • Do not empty your bladder before your appointment.
