When you choose a Cone Health-affiliated primary care physician or advanced practice provider, you gain a long-term partner in health. Your PCP gets to know you, your health history and your personal goals as you visit regularly for preventive care and treatment of a wide range of illnesses and medical conditions.
What Does a PCP Do?
Your PCP is your main health care provider. That means he or she should be the person you call for non-emergency medical care. Count on your PCP for:
- Preventive care, including annual physicals, routine health screenings and immunizations
- Diagnosis and treatment of common conditions
- Assistance managing chronic conditions
- Referrals to specialty care when needed
- Coordination of care across settings and providers

Extension of Care
Within primary care, Cone Health is proud to offer services that extend a patient’s care based on their specific needs. This care exists to support both Medicare and non-Medicare patients who can benefit from our assistance in managing a diagnosis or health-related need.
Four specific services are available through this extension of care program:
- Annual Wellness Visits: Medicare patients can connect with their provider each year for an overall health and wellness check.
- Chronic Care Management: Chronic Care Management offers personalized support to Medicare patients who suffer from two or more chronic conditions.
- Care Coordination: This program offers care coordination, disease management and education, and support to remove barriers to meeting health goals.
- Community Resource Care Guides: Our Care Guides help patients get past hurtles related to their health or lifestyle.
