Diabetes Prevention Program
The Cone Health Medicare Diabetes Prevention Program helps you change your lifestyle to prevent you from getting type-2 diabetes. You’ll reach your goals with the help of a trained lifestyle coach and a small group of people who are all committed to a healthier future.
Group Education for Type 2 Diabetes
After a diagnosis of type 2 diabetes or prediabetes, join your peers for a comprehensive ADA-recognized diabetes health management education class. You’re encouraged to bring a support person with you. You’ll meet once a week for three weeks to learn about:
- Basic diabetes education – Review what it means to live with diabetes and set personal health goals
- Nutrition management – Plan meals that fit your preferences and health needs
- Activities, maintenance and medications – Discuss stress management and how to achieve long-term goals
