Your First Visit
Before your rehab journey with us officially starts, your first visit will be an orientation.
This takes about 90 minutes and will help set you up for success during your time with us. You will meet one on one with a care team member to talk about your goals and your well-being, and they will review your patient questionnaire with you.
This is a great time for you as the patient to talk about health or psychosocial concerns. You will then work with an exercise physiologist to do a physical assessment, which includes a six-minute walk test, flexibility test, grip strength test and body mass measurements.
Before this orientation visit, a care team members will contact you to get a medication list and medical history to create a patient profile for you.
