Cone Health Community Health & Wellness Center is a patient-centered practice. We provide integrated care in an effort to achieve positive patient experiences and improve patient health care outcomes. Our team is here to support patients in their wellness journeys as well as to provide chronic disease management, lab testing, health education and much more. Free valet parking is available for patients at the Olive Street entrance.

Same-Day & Walk-In Appointments

Cone Health Community Health & Wellness Center has same-day and walk-in appointments available for established patients only. Walk-in appointments are available on Wednesday and Thursday from 8:30 am to 11:00 AM and from 1:30 AM to 4:00 PM. Same-day appointments are available daily from 8:30 AM to 4:15 PM.

Use MyChart to view medical test results, check your payment balance, contact your provider, request prescription refills and more. 

Appointment Guidelines

  • Before your scheduled appointment with us, you will receive two reminder phone calls.
  • Arrive 15 minutes early for your appointment. Bring your insurance card and a list of your current medications.
  • If you need to cancel or reschedule your appointment, please do so within 24 hours of your appointment to avoid any fees.

After-Hours Calls

For assistance after hours, call our office’s main number – (336) 832-4444 – and you’ll be transferred to our after-hours call center for help.

Payment Information

  • In most cases, you are expected to pay in full or pay your full co-payment for all office services at the time of your appointment.
  • If you cannot provide proof of insurance, you will be totally responsible for payment of the bill at the time of your visit.
  • Financial assistance is available to those without insurance.
