Advance Directives are legal instructions regarding your healthcare wishes. They help you have a say about how you want to be cared for if you cannot speak for yourself.
- Living Will: A document which directs your medical team on the types of life-prolonging medical treatments you prefer if your condition will not improve.
- Health Care Power of Attorney: A document which allows you to choose someone (your "agent") to make medical decisions for you if you become unable to make those decisions for yourself.
A team member will be available to review documents with you, and a notary will be present to help you sign and complete the forms. (Note: Advance Directives DO NOT address finances, property, or other legal advice.)
