Appointment Guidelines

  • Before your scheduled appointment with us, you will receive a reminder phone call.
  • Arrive 15 minutes early for your appointment. Bring your insurance card and a list of your current medications.
  • If you need to cancel or reschedule your appointment, please make sure to do so 48 hours or more in advance of your scheduled appointment to avoid a fee and allow the appointment to be given to another patient in need.

Payment Information

  • In most cases, you are expected to pay in full or pay your full co-payment for all office services at the time of your appointment.
  • If you cannot provide proof of insurance, you will be totally responsible for payment of the bill at the time of your visit.

Insurance Information

Many health insurance plans will not cover cosmetic surgeries and treatments, however, reconstructive and medical related procedures may be covered. Review your health insurance policy and talk to them about your coverage to understand what portion of the expenses you will be responsible for.
