If you receive care at a hospital in the Cone Health network, your team may include hospitalists—doctors who specialize in caring for you while you’re in the hospital.

You’ll get 24/7 on-site care from collaborative board-certified physicians who have experience managing complex and urgent medical conditions. Plus, you can count on efficient care because our hospitalists know our hospitals’ resources and services well.

Your Quarterback for Hospital Care

Your hospitalist is like the quarterback of your hospital care team. That’s because hospitalists:

  • Order tests and explain the results
  • Help you and your family understand your diagnosis and treatment options
  • Coordinate care with specialists as needed
  • Monitor your progress and update your care plan as needed
  • Keep your primary care provider up-to-date on your care and condition
  • Help arrange follow-up visits and rehabilitation services
  • Address other concerns as needed

Coordinated, Consistent Care

You’re assured of safe, consistent care in and out of the hospital because our hospitalists communicate with your primary care provider throughout your stay.

When you’re admitted to the hospital, we’ll review your medical history as well as information provided by you and your family. When you leave the hospital, we’ll send a detailed summary of your hospital stay to your primary care doctor.

With Cone HealthLink, your hospitalists, primary care provider and specialists have secure, immediate access to your Cone Health medical record so they’ll always have a current, complete picture of your health.

Who Do I See For Follow-Up Care?

Before you leave the hospital, your hospitalist will schedule your follow-up appointment with your primary care provider and, if needed, your specialist.

If you don’t have a primary care provider, we’ll refer you to one. See your primary care provider regularly to stay as healthy as possible.
