1. Log into MyChart.
     Don't have a MyChart account? Register here
2. Start an e-Visit
3. Select a reason for the e-Visit.
4. Complete the questionnaire in 15 minutes and receive a care plan in less than an hour.

Please note that providers will not be able to provide FMLA forms, disability forms, or handicap/DWV documents as these documents require in-person evaluation.
